Eligible Equipment

Examples of eligible items include, but are not limited to the following

Mobility aids

  • Wheelchairs: Manual or power wheelchairs to assist with mobility.

  • Mobility scooters: Electric scooters for increased independence in outdoor activities.

Orthopedic Supports

  • Orthotic devices: Braces or orthopedic supports to provide stability and support for weakened muscles.

Respiratory Equipment

  • BiPAP/CPAP machines: Devices to assist with breathing and ensure proper ventilation during sleep.

  • Cough assist devices: Devices to aid in clearing secretions from the airways.

Transfer Aids

  • Transfer boards: Aids to assist in transferring from one surface to another.

  • Patient lifts: Devices to help transfer individuals between surfaces with minimal physical effort.

Eligibility

Application Requirements

  • Applicants must provide 1-2 quotes from the selected vendor for the specific item(s) requested. The quote(s) must precisely match the item(s) specified in the application. Please note that applications lacking exact quotations will be considered incomplete. Approved applications will be fulfilled with the exact item(s) as quoted; therefore, changes to the requested item(s) cannot be accommodated after the review process.

  • Applicants are required to submit a completed Diagnosis Verification Form. This form must be certified by a licensed nurse practitioner or primary physician, confirming the medical necessity of the requested item(s).

  • Applicants must provide a copy of their most recently filed tax return or Supplemental Security Income (SSI) statement. This documentation should cover either the applicant individually or the entire household.

  • Applicants must provide a 250 word essay addressing the following prompts:
    Why is ___ needed? How will ___ improve your life? (Note: "___" represents the specific item being requested.)

Terms & Conditions

  • Grants awarded through the IDISCY Medical Equipment Fund are disbursed directly to the selected supplier/vendors of the equipment. Applicants must provide 1-2 estimates from their chosen vendor. Please note that the IDISCY Foundation does not provide cash grants for reimbursement of previously purchased equipment.

  • Applicants are required to specify the modifications or equipment needed when applying to the IDISCY Medical Equipment Fund. Upon approval of the application, the IDISCY Foundation cannot accommodate returns or exchanges for the provided equipment.

  • Recipients must complete a survey sent via email within 90 days of receiving the item(s). This brief survey, lasting approximately 5 minutes, is crucial for assessing the impact of the provided equipment.

  • The IDISCY Medical Equipment Fund is open only to individuals who reside in and are citizens of the United States.

  • Winners will be determined by an impartial IDISCY Foundation Review Committee, and the decision is final. Selection criteria will be based on the essay and application requirements.

  • The IDISCY Foundation does not discriminate based on race, religion, gender, sexual orientation, age, disability, or any other protected status.

  • Applicants who are not chosen are encouraged to reapply during the next application cycle.

We carefully curate a list of categories for donations to support. The exact allocation to each category is on a per-need basis that we deem the most effective use for our charity goals in helping others with Muscular Dystrophy.
